Once you schedule your appointment, you will receive a confirmation email as well as reminder emails and/or texts for your appointments. 

​

Shortly after making your appointment, you will receive a link to complete your new patient intake forms - all which will be completed online before your appointment. 

​

During the Covid-19 pandemic, you will also receive a covid-19 screening form to complete the day of your appointment. Please check for this - it should arrive 4 hours before your appointment start time.
