Knee Pain Is the Body's Most Common Joint Complaint

Whether it's the grinding ache of osteoarthritis in a 55-year-old who spent decades on their feet, the sudden swelling after a weekend hiker's overreach, or the chronic patellar pain in a 30-year-old desk worker whose hips have stopped working properly, knee pain is one of the most frequent reasons people seek care outside the conventional system.

It's also one of the conditions where conventional medicine offers a frustrating middle ground: anti-inflammatories that work short-term but accelerate cartilage damage long-term; cortisone injections that provide temporary relief but lose efficacy with repetition; physical therapy that helps some patients dramatically and others not at all; and surgery that ranges from genuinely appropriate to wildly overutilized, depending on who you're seeing and where.

Acupuncture for knee pain is not fringe. It's backed by more rigorous evidence than most orthopedic interventions that get billed at ten times the price, and it works through mechanisms we now understand reasonably well.

The Anatomy of Knee Pain

The knee is a hinge joint that relies on dynamic muscular support more than almost any other joint in the body. Four main structures bear the load: the medial and lateral compartments of the tibiofemoral joint (where thigh meets shin), the patellofemoral joint (where the kneecap glides), the menisci (shock-absorbing cartilage discs), and the ligamentous complex (ACL, PCL, MCL, LCL) that restrains the joint through its range of motion.

The knee is highly dependent on what happens above and below it. Hip abductor weakness causes medial collapse — the knee caving inward — that loads the medial compartment asymmetrically and accelerates cartilage wear. Tight calves and restricted ankle dorsiflexion change how load is transferred up the kinetic chain, shifting stress onto the knee during gait. Flat arches change pronation patterns that show up miles above at the knee. The knee takes the hit for whatever the hip and foot are not doing properly.

Knee osteoarthritis is the most common form of knee pain in adults over 45. It involves progressive erosion of the articular cartilage, subchondral bone remodeling, osteophyte (bone spur) formation, synovial inflammation, and changes in periarticular soft tissues. But the severity of the X-ray and the severity of the pain correlate poorly — some patients with significant radiographic arthritis report minimal symptoms, while others with mild imaging findings are functionally disabled. The pain is driven not only by structural changes but by local neuroinflammation, sensitization of joint afferents, and central pain processing changes that develop over time.

The TCM View of Knee Pain

Chinese medicine classifies most knee pain under Bi Syndrome — painful obstruction of the channels by Wind, Cold, Dampness, or Heat, often superimposed on underlying deficiency of the Liver and Kidney organ systems.

Liver and Kidney Deficiency. The Kidney governs bones and marrow; the Liver governs tendons. Decline in these organ systems — which in biomedical terms maps onto age-related hormonal and metabolic shifts, nutrient depletion, and constitutional vulnerability — is understood as the root of degenerative joint disease. Symptoms include aching, weakness, and slow-onset stiffness that worsens with fatigue.

Wind-Cold-Damp Bi. Pain that is worse in cold or wet weather, improved by warmth, with a sense of heaviness or stiffness in the joint. This pattern corresponds well to the weather-sensitivity that many arthritis patients report.

Damp-Heat Bi. Swelling, heat, and redness in the joint — the inflammatory flare pattern. This correlates with the hot, swollen knee of active inflammatory arthritis.

Blood Stasis. Fixed, sharp, or stabbing pain in a specific location, often with a history of injury. Corresponds to local circulatory impairment and tissue adhesion.

How We Treat Knee Pain at GoodMedizen

We treat the knee as part of a kinetic chain, not an isolated joint. This means:

Local acupuncture and dry needling. Points around the knee — including ST35 (Dubi), ST36 (Zusanli), SP9, SP10, GB34 — address local inflammation, fluid accumulation, and joint afferent sensitization. Myofascial trigger points in the quadriceps, IT band, and popliteal muscles are treated as part of the same session. Evidence: multiple Cochrane reviews and meta-analyses confirm acupuncture produces clinically meaningful reductions in pain and WOMAC scores, with effects sustained for up to 12–24 months in some trials.

Our TRACS system. Our proprietary Tissue Response Assessment and Corrective Strategy approach applies to knee pain by systematically identifying and treating the trigger points and fascial restrictions in the hip, thigh, calf, and foot that load the knee asymmetrically. Treating the knee without correcting hip abductor weakness or restricted ankle mobility is treating the symptom and not the problem.

Electroacupuncture. EA enhances the local anti-inflammatory and analgesic effect, particularly for moderate-to-severe arthritis. Studies show EA outperforms manual acupuncture alone for both pain and functional scores.

Point Injection Therapy (PIT). For stubborn knee pain that has not responded adequately to needling alone, targeted injections — procaine or lidocaine for trigger point deactivation, homeopathic Traumeel or Zeel for joint inflammation, B12 for neuropathic components — can produce relief that outlasts the pharmacological half-life by weeks. This is one of our most consistently effective tools for chronic knee pain.

Chinese herbal medicine. Internally, Duhuo Jisheng Tang and related Kidney-and-Liver-tonifying formulas have the strongest evidence base among TCM interventions for knee osteoarthritis. They reduce inflammation, support cartilage, and address the deficiency root. Topically, herbal liniments improve local circulation and reduce pain acutely.

Functional medicine assessment. Systemic inflammation matters for joint health. We evaluate hs-CRP, uric acid, metabolic drivers of inflammation, and nutritional factors — particularly vitamin D, omega-3 status, and collagen precursors. Joint health doesn't exist in metabolic isolation.

When to Consider Us

  • You have a knee osteoarthritis diagnosis and want to reduce pain and improve function before considering surgery
  • You've had knee surgery and want to accelerate recovery and prevent re-injury
  • Your knee pain is linked to activity level, weight, or sports and you want comprehensive care
  • Cortisone injections have stopped working or you want to reduce reliance on them
  • You have chronic swelling, stiffness, or grinding that limits your daily life
  • Your pain is worsened by cold, weather changes, or fatigue
